Requires Lyme disease to be included on the Michigan Department of Health and Human Services' list of reportable diseases, infections, and disabilities.

Mandates that licensed health professionals and health facilities report Lyme disease diagnoses to the department or local health department within 24 hours of diagnosis on a form provided by the department.

Authorizes the department to issue warning notices for first violations of Lyme disease reporting rules and assess civil fines of $10 to $100 for second and subsequent violations.

Makes Lyme disease reporting requirements subject to the same surveillance and reporting procedures established for other communicable and serious communicable diseases.
