Requires the Department of Health and Human Services to establish, maintain, and revise a uniform community mental health services credentialing program compliant with national standards.

State departments and agencies providing community mental health services must comply with the uniform credentialing program and use the department's provider information profile once the program is certified as in full force and effect.

Requires credentialing and recredentialing documentation for physicians, physician's assistants, psychologists, social workers, licensed professional counselors, nurses, occupational and physical therapists, and speech language pathologists.

Exempts health plans under contract with the state for Medicaid services and health plans regulated by the insurance code from the uniform credentialing requirement; these entities may use plan-specific processes.

Requires the department to submit annual reports to the legislature describing activities and revisions to the uniform credentialing program, with the first report due 6 months after the law's effective date of March 24, 2021.
