Amends the Public Health Code to require immunization certificates include a space to indicate whether a minor has been screened for lead poisoning, effective January 1, 2021.

Changes statutory language from "shall" to "must" for certificate requirements regarding diseases, doses, dates, and future immunizations needed.

Clarifies that parents, guardians, or persons in loco parentis must be notified of their right to object to the immunization reporting requirement before a provider reports to the department.

Protects health care providers from civil liability for good faith compliance or non-compliance with reporting requirements, except for gross negligence or willful and wanton misconduct.

Bill takes effect only if Senate Bill or House Bill 5365 is enacted into law.
