Defines "bona fide prescriber-patient relationship" as requiring a prescriber to review patient medical records and complete an assessment including a medical evaluation conducted in person or through telehealth.

Requires prescribers to create and maintain records of patient conditions in accordance with medically accepted standards as part of establishing a valid prescriber-patient relationship.

Amends definitions in the Public Health Code related to controlled substances, controlled substance analogues, counterfeit prescription forms, counterfeit substances, and electronic signatures.

Takes effect July 8, 2019, contingent upon enactment of House Bill No. 4225.
