• Requires the Department of Health and Human Services to forward completed report forms to the identified law enforcement agency, or to the Michigan State Police if no agency is identified.

• Makes the State Police responsible for identifying the employing law enforcement agency when not listed on the form and notifying the victim if the agency cannot be identified.
