Requires the Department of Corrections to create a prerelease mental health discharge plan for each prisoner receiving mental health services or medication before parole release.

Discharge plans must include mental health assessment using standardized tools (Patient Health Questionnaire-9, Generalized Anxiety Disorder 7-Item Scale, Patient Health Questionnaire-2, and Opioid Risk Tool), current diagnosis, prognosis, and medications.

Plans must identify risk factors related to employment, violence history, transportation, housing, and family stress; schedule a post-release mental health appointment; and assess Medicaid or Medicare eligibility with enrollment assistance if applicable.

Authorizes use of cloud-based care management software meeting specified security and functionality requirements, including 2-factor authentication, natural language processing, and integration with the state's eligibility system.

Takes effect 90 days after enactment; allows prisoners to decline planning (which must be noted in their record).
