Amends section 5145 of the Public Health Code to require the Department of Health and Human Services to develop reports, implement policies, and establish processes related to nursing home coronavirus response, including visitation policies, diagnostic testing coordination, and care and recovery centers.

Requires the department to establish a website assigning risk levels (low, A, B, C, D, or E) to each county and region based on daily test positivity rates, with low risk at less than 3% positivity and level E at greater than 20% positivity.

Mandates implementation of a statewide nursing home visitation policy allowing in-person indoor and outdoor visits, with limitations only for residents who test positive, facilities experiencing outbreaks, or communities with active coronavirus transmission.

Establishes requirements for care and recovery centers within nursing homes for coronavirus-positive individuals, including minimum 3-star rating, dedicated staffing, adequate personal protective equipment, and medical clearance for transfers.

Requires any updates to visitation policies based on county or regional risk levels must use only the test positivity rate-based risk levels defined in the bill, not other metrics.
