Amends Public Health Code section 9206 to require immunization certificates include a space indicating whether a minor has been screened for lead poisoning, effective January 1, 2022.

Changes mandatory language in certificate requirements from "shall" to "must" for clarity regarding form prescription by the department and required immunization information.

Clarifies parental notification requirements by replacing "of" with "described in" when referring to the reporting requirement objection process.

Clarifies that if a parent, guardian, or person in loco parentis objects in writing prior to notification, the health care provider shall not report the immunization.

Enactment is contingent upon passage of Senate Bill No. ____ or House Bill No. 4678 of the 101st Legislature.
