Prohibits state agencies from requiring individuals to present documentation of COVID-19 vaccination or immunity status to access state property or receive state services, except as required by federal or state law.

Defines "state agency" to include the state and all departments, agencies, boards, commissions, and other instrumentalities or entities of the state.

Allows individuals to bring civil actions against state agencies for violations, with damages escalating based on number of prior judgments: $1,000 for first violation, $10,000 for second, $100,000 for third, and $1,000,000 for fourth or subsequent violations.

Requires courts to award reasonable attorney fees to individuals who prevail in civil actions against state agencies.
