Requires disease reporting rules under the Public Health Code to allow individuals with communicable diseases, serious communicable diseases or infections, or disabilities to object to reporting their name or other identifying information.

Mandates that persons making disease reports notify individuals of their right to object to having their name or identifying information included in the report, using a department-provided form.

Allows individuals whose identifying information was already included in a disease report to request that the department make that information inaccessible in any database it maintains.

Applies these requirements to all rules promulgated by the Department of Health and Human Services regarding reporting of diseases, infections, and disabilities to the state or local health departments.
