Candidates filing for federal, state, county, city, township, village, or school district office must file 2 copies of an affidavit of identity with the filing officer, or within 1 business day if nominated at a political party convention or caucus (exemption for Presidential and Vice Presidential candidates)

Affidavit of identity must include candidate's name, residential address, citizenship status, office sought with jurisdiction, political party affiliation, constitutional/statutory qualifications statement, and desired ballot name format

Candidates using a name not given at birth must disclose their full former name on the affidavit, except for names changed 10+ years ago, through naturalization, marriage, divorce, or under common law name provisions

Affidavit must include a notarized statement confirming all campaign finance reports, statements, late fees, and fines have been filed or paid under the Michigan campaign finance act, with perjury penalties up to $1,000 fine or 5 years imprisonment

Officers must not certify candidates who fail to file the affidavit or who file one containing false statements; county clerks must forward state and federal candidate affidavits to the Secretary of State
