Requires absent voter ballot applicants to provide driver license number, official state personal identification card number, last 4 digits of Social Security number, or present/attach identification for election purposes when applying for an absent voter ballot.

Establishes provisional absent voter ballots processed under section 523a(5) for applicants who do not provide required identification, which are subject to verification and not tabulated on election day unless identity is later confirmed.

Adds new section 760a requiring city and township clerks to have access to a Department of State data system containing Social Security numbers and driver/identification card numbers to verify elector identity.

Applies identification requirements to all absent voter ballot applications including those from uniformed services voters, overseas voters, and emergency absent voter applicants under section 759b.

Updates absent voter ballot application forms and instructions to include voter identification options and clarifies that applicants who fail to provide identification will receive provisional ballots requiring verification.
