Amends section 495 of the Michigan Election Law (1954 PA 116) regarding voter registration applications.

Requires voter registration applications to include a statement informing applicants that it is a felony to offer to vote or attempt to vote more than once at the same election in the same or another voting precinct.

Maintains all existing registration application requirements including name, address, date of birth, citizenship status, age verification, and residency information.

Applies to voter registration applications processed by city and township clerks in Michigan.
