Removes the prohibition that prevented cities with populations greater than 600,000 from exempting themselves from the Publicly Funded Health Insurance Contribution Act requirements.

Allows local units of government to exempt themselves from the act's requirements for one year by a 2/3 vote of their governing body before the medical benefit plan coverage year begins.

Requires a 2/3 vote each year to extend an exemption for succeeding medical benefit plan coverage years.

Exemptions for cities with mayors serving as both chief executive and chief administrator require mayoral approval.

Exemptions for counties with county executives serving as both chief executive and chief administrator require county executive approval.
