Adds a new method for individuals to make anatomical gifts by completing and filing a donor registry schedule with their annual state income tax return under the income tax act of 1967.

Requires the Department of Treasury to transmit donor registry schedules filed by individuals to the Secretary of State, who must add those individuals to the state's donor registry.

Maintains existing anatomical gift methods including driver license/identification card designation, wills, communication during terminal illness, and donor cards or registry records.

Requires the Secretary of State to maintain the donor registry with electronic access available 24/7 at no cost to organ procurement organizations, tissue banks, and eye banks to verify anatomical gifts at or near death.

Takes effect only if House Bills 4362 and 4364 of the 102nd Legislature are also enacted; effective immediately upon enactment.
