Requires the Department of Health and Human Services to collaborate with contracted health plans to create incentives for Medicaid recipients who practice positive health behaviors, including participation in health risk assessments, smoking cessation, immunizations, prenatal visits, and attendance at medical appointments.

Establishes pay-for-performance incentives for Medicaid health maintenance organizations to meet health outcome targets for chronic disease states and patient compliance, with priority given to strategies addressing the 10 most prevalent and costly conditions affecting Medicaid recipients.

Directs the department to establish a preferred product and service formulary program for durable medical equipment, with potential joint partnership with Centers for Medicare and Medicaid Services to achieve savings and efficiencies.

Authorizes the department to seek Medicaid funding for electronic health records initiatives, including personal health records, e-prescribing, web-based medical records, and other health information technology.

Requires the department to use Medicaid funds for specialty services and supports for beneficiaries with serious mental illness, developmental disabilities, serious emotional disturbance, or substance use disorders, delivered through specialty prepaid health plans carved out from basic Medicaid benefits.

Repeals sections 105c and 105f of the 1939 Social Welfare Act and becomes effective immediately upon approval on July 19, 2023.
