Amends sections 10121 and 10301 of the Public Health Code to update references to powers of attorney under the Uniform Power of Attorney Act and patient advocate designations under the Estates and Protected Individuals Code.

Modifies section 10121 to clarify that advance health care directives include durable powers of attorney and patient advocate designations, and establishes procedures for resolving conflicts between end-of-life declarations and anatomical gift terms.

Updates section 10301 regarding the Peace of Mind Registry to allow directives to be submitted by mail or secure electronic format (PDF or other department-approved formats) and requires directives to include a signature line.

Expands Peace of Mind Registry requirements to include electronic access for health care providers at no cost, unique identifier-searchable database using last 4 Social Security number digits and date of birth, and annual reporting to legislative health policy committees on registrant numbers.

Takes effect only upon enactment of House Bill 4644 of the 102nd Legislature and provides liability immunity for Peace of Mind Registry organizations except for willful or gross negligence.
