Maintains provisional license fees at the same rate as regular licenses, with no additional fee charged when converting to a permanent license within the 1-year period.

Establishes construction permit fee at $300.00 for mental health facilities.

Specifies that denied applications and revoked licenses result in forfeiture of all fees paid to the department.
