Requires health insurance policies to cover a 12-month supply of prescription contraceptives dispensed at one time from network pharmacies, unless the insured requests a smaller supply or the prescriber specifies otherwise.

Mandates coverage for outpatient consultations, examinations, procedures, and medical services necessary to prescribe, dispense, deliver, distribute, administer, or remove prescription contraceptives if other prescription drugs are covered.

Allows insurers to apply standard copayments, deductibles, and coinsurance to contraceptive coverage but prohibits utilization controls or medical management that limits supply below 12 months unless the FDA determines such supply is medically inappropriate.

Exempts insurers from providing 12-month supply coverage during the one-month period before an insured's plan year ends.

Applies to health insurance policies delivered, executed, issued, amended, adjusted, or renewed in Michigan beginning December 31, 2025.
