Establishes a statewide perinatal quality collaborative within the Department of Health to improve maternal and infant health outcomes through quality improvement efforts, resource mobilization, equity advancement, and expanded care for perinatal substance use disorder.

Creates regional perinatal quality collaboratives for each of Michigan's 10 prosperity regions, with each region designating a lead agency to coordinate participation from qualified persons and interested stakeholders.

Requires regional collaboratives to convene regularly to review maternal and infant health data, develop action plans to address region-specific perinatal challenges, and engage families and communities in planning efforts.

Defines "qualified person" as providers and entities serving pregnant individuals and infants, including health facilities, health professionals, local health departments, home visitation programs, insurers, community-based organizations, and federally recognized tribes.

Takes effect April 2, 2025; requires appropriations to provide resources to regional collaboratives.
