Establishes a transition bridge program overseen by the office of mental health and suicide prevention within the Michigan veterans affairs agency to provide mental health, well-being, and suicide prevention services for service members and veterans.

Requires the program to be offered at least 4 times per year and available within 2 months before and after separation from military service, with eligible members able to enroll within 1 year of separation (waivable by the office manager).

Program curriculum must include warning signs for PTSD, depression, substance use disorder, and suicide; traditional and alternative mental health treatment options; and guidance on transitioning from military to civilian life.

Eligible participants include Michigan residents who currently serve or are veterans of the Michigan National Guard, U.S. Armed Forces (active duty or reserve), or other uniformed services (NOAA or Public Health Service commissioned officers).

Effective only upon enactment of House Bills 5276, 5277, 5279, and 5720; individual service member data collected is exempt from public disclosure under the Freedom of Information Act.
