Amends the Public Health Code to allow individuals who indicate willingness to be organ donors on an income tax return schedule to be added to Michigan's donor registry.

Requires the Department of Treasury to transmit donor registry schedules filed by individuals to the Secretary of State, who maintains the registry and provides electronic access to organ procurement organizations, tissue banks, and eye banks 24/7 at no cost.

If a donor registry schedule includes a driver license or state personal identification card number, the Secretary of State adds that individual to the donor registry; if not, the Secretary of State provides the data directly to the organ procurement organization.

Establishes that personally identifiable information on the donor registry cannot be used or disclosed without express consent except to determine whether a donor has made, amended, or revoked an anatomical gift at or near time of death.

Permits private donor registries not established by the state, provided they comply with privacy protections, notify the organ procurement organization within 30 days of establishment, and grant full access to anatomical gift records.
