Requires the Michigan Department of Corrections to provide prisoners with medical information release forms, emergency contact forms, and an informational brochure upon intake or by the effective date for current prisoners.

Allows prisoners to designate authorized recipients to receive their qualified medical information and emergency contacts to be notified during critical medical emergencies requiring hospitalization.

Mandates the department immediately notify a designated emergency contact when a prisoner experiences a critical medical emergency and may arrange hospital visits by approved family members or clergy unless a safety concern exists.

Requires prisoners to maintain accurate contact information on their forms and allows them to request amendments at any time.

Directs the department to develop the forms within 90 days and create an informational brochure within 60 days of the effective date, with the brochure made available on the department's website and at annual health examinations.
