Requires physicians to use a standardized medical contraindication form created by the Department of Health and Human Services when certifying that a specific immunization is detrimental to a child's health or inappropriate.

Mandates that parents, guardians, or persons in loco parentis obtain the nonmedical immunization waiver form in person from a local health department and have their signature witnessed by a local health officer before claiming a religious or personal objection exemption.

Requires local health officers to inform parents about the risks of not receiving waived vaccines and the benefits of immunizations to the child and community before witnessing the waiver form signature.

Specifies required contents for both the medical contraindication form (physician information, vaccine identification, expiration date of exemption) and the nonmedical waiver form (child information, objection description, acknowledgment of disease risk).

Bill takes effect only if Senate Bill No. ____ or House Bill 6206 of the 102nd Legislature is also enacted into law.
