Removes the 2014-2015 school year enrollment requirement, allowing children enrolling in grade 7 for the first time (rather than only those beginning in 2014-2015) to submit immunization documentation.

Replaces religious conviction exemptions with requirements to submit signed medical contraindication forms or nonmedical immunization waiver forms as specified in the Public Health Code (MCL 333.9215).

Updates department name from "Department of Community Health" to "Department of Health and Human Services" throughout the statute.

Changes reporting language from "shall" to "must" and updates grade range descriptions from "through" to "to" for consistency in immunization status reports due November 1 and February 1 annually.

Makes the act contingent on enactment of either Senate Bill No. ____ or House Bill No. 6205 (tied bill requirement).
