Requires local health officers to submit written reports to local governing entities by June 1, 2026 and annually thereafter detailing immunization levels of children entering school

Reports must include the percentage of children exempt from immunization under both section 9215(1) and section 9215(2), and identify schools where exemptions exceed 5% of total students

Schools with exemption rates exceeding 5% must include in the report a description of actions the local health department plans to take in the next year to reduce exemptions below 5%

Makes clarifying language changes by replacing "pursuant to" with "under" and "shall" with "must" throughout the existing immunization reporting requirements

Adds definition of "local governing entity" as that term is defined in section 2406 of the public health code
