Creates Part 128 establishing comprehensive regulation of onsite wastewater treatment systems by the Department of Environment, Great Lakes, and Energy and local health departments, including standards for conventional and alternative systems.

Requires owners of premises not connected to public sanitary sewers to obtain onsite wastewater treatment system evaluations on specified timelines: within 10 years for systems 20+ years old near surface water, before changes in use, and every 10 years thereafter after first evaluation.

Establishes registration requirements for proprietary wastewater treatment products at $3,000 application fee and for evaluators at $180 application fee, with $50 state administrative fee per evaluation charged to property owners.

Creates the Onsite Wastewater Treatment System Administration Fund and Public Education and Training Fund through fees and fines to support program administration, training, and public education.

Imposes civil fines up to $250-$500 monthly for failing to obtain required evaluations and up to $1,000 per 30-day period for failure to remedy system failures, with liens recordable on real property after 2 years of unpaid fines; exempts owners below 400% of federal poverty level.
