Requires health insurers delivering or renewing policies in Michigan to provide written summaries in plain English describing coverage, cost-sharing, exceptions, and limitations in accordance with federal guidance.

Mandates insurers include specific information in policy summaries including uniform definitions, coverage examples, minimum essential coverage status, network provider access instructions, and formulary information.

Requires insurers to provide summaries to applicants within 7 business days of application, by first coverage date if information changes, within 30 days of renewal, and within 7 days of request.

Establishes requirements for insurers to disclose provider credentials, network information, prior authorization requirements, and financial relationships with providers upon insured request.

Takes effect only if Senate Bill No. 633 of the 102nd Legislature is enacted into law.
