Expands immunization reporting requirements to include students enrolling in grade 12 for the first time beginning in the 2025-2026 school year, in addition to existing requirements for first-time enrollees and grade 7 first-time enrollees.

Requires school administrators to report immunization status and vision screening information to the state Department of Health and Human Services by November 1 and February 1 of each year for affected students.

Updates statutory language to refer to the "Department of Health and Human Services" instead of "Department of Community Health" throughout the section.

Makes immunization reporting rules subject to the public health code provisions regarding exemptions, specifically referencing MCL 333.9227.

Takes effect only if Senate Bill No. 875 of the 102nd Legislature is enacted into law.
