Requires voters without identification for election purposes to sign an affidavit and present one document proving identity and one document proving state residency to vote a regular ballot.

If a voter signs an affidavit but cannot provide both proof of identity and proof of state residency, that voter receives a provisional ballot that is not tabulated on election day but secured for verification afterward.

Defines acceptable documents for proof of identity (marriage license, birth certificate, military discharge, driver education certificate, etc.) and proof of state residency (utility bills, bank statements, lease agreements, pay stubs, insurance policies, vehicle registration, etc.).

Allows provisional ballots cast by voters who lack required documentation to be tabulated after the election if the voter later provides the required proof of identity and proof of state residency documents to the city or township clerk within 6 days of the election.

Requires city, township, and county clerks to report provisional ballot and affidavit statistics to the secretary of state within specified timeframes for public inspection.
