Requires mental health providers to communicate patient rights orally and in writing upon commencement of treatment, rather than orally only.

Mandates that communications include the right to end voluntary mental health treatment and the process for doing so.

Requires providers to give copies of written consent and the form described under section 1419 to the patient, the individual who executed consent, and one other individual designated by the patient.

Specifies that the section 1419 form must be provided even if the patient has not expressed an intention to terminate treatment.
