Requires health insurance policies that cover prescription contraceptives to provide coverage for a 12-month supply dispensed at one time from an in-network pharmacy, unless the insured requests or prescriber instructs a smaller supply

Mandates coverage for outpatient consultations, examinations, procedures, and medical services necessary to prescribe, dispense, administer, or remove prescription contraceptives

Prohibits insurers from imposing utilization controls or medical management that limits contraceptive supply to less than 12 months, unless the FDA determines a 12-month supply is medically inappropriate

Allows insurers to apply standard cost-sharing provisions (copayments, deductibles, coinsurance) that apply equally to other covered prescription drugs

Applies to health insurance policies delivered, issued, amended, or renewed beginning December 31, 2025
