Mandates providers report specific data on service shutoffs, including total number of shutoffs, shutoff notices issued to residential dwellings, reasons for shutoffs, and time between shutoff and service restoration broken down by census tract and zip code

Requires providers to report on low-income assistance programs, including enrollment numbers, benefit levels, eligibility criteria, funding sources, program costs, and default rates

Obligates providers to maintain a public website with current rate schedules, shutoff policies and procedures, payment plan information, affordability programs, and copies of annual reports

Directs the Department to develop a standardized reporting form and online portal, make all submitted reports publicly available in a searchable format, and post names of non-compliant providers on the department's website
