Requires health care professionals who perform abortions to report each procedure to the Michigan Department of Health within 7 days on prescribed forms

Mandates collection of specific demographic and medical data including patient age, marital status, race, residence, gestational period, abortion method, facility information, payment method, and reasons for obtaining the abortion

Prohibits including patient names, Social Security numbers, or any identifying information in reports; state agencies cannot cross-reference data that could identify patients

Establishes criminal penalties of up to 3 years imprisonment and/or $5,000 fine for unauthorized disclosure of confidential identifying information

Requires the department to publish annual aggregate statistical reports summarizing abortion data, including gestational periods, procedures on minors, and physical complications

Individual reports must be destroyed after 5 years; bill is contingent on passage of HB 5202 or SB ____ (S02115'25)
