Requires health care professionals to file written reports with the department for each patient suffering from physical complications or death resulting from an abortion (as primary, secondary, or tertiary cause)

Mandates the department to include aggregate data from these reports in the annual statistical report on abortion required under section 2835

Requires destruction of individual reports after 5 years of retention

Prohibits including patient names, addresses, or other identifying information on the standardized reporting form

Requires distribution of the standardized form to all urgent care facilities and emergency departments statewide, with public availability on the department's website

Bill only takes effect if companion bill HB 5201 or SB ____ (request no. S04533'25) is also enacted
