Requires the Department of Corrections to provide prisoners with medical information release forms and emergency contact forms upon arrival at a correctional facility, as well as an informational brochure

Mandates the department immediately notify a prisoner's designated emergency contact when the prisoner experiences a critical medical emergency requiring hospital inpatient care

Allows approved visitors (department-approved immediate family members and clergy) to visit hospitalized prisoners during critical medical emergencies if the treating physician and department determine it necessary

Requires the department to release a prisoner's selected medical information to their designated authorized recipient upon request

Requires the department to develop the necessary forms within 90 days and create an informational brochure within 60 days of the act's effective date, with the brochure made available online and during annual health examinations
