Updates terminology from "physician" to "qualified health care professional" for signing immunization statements and compliance certifications

Requires all immunization status information reported by school administrators to the state be deidentified and aggregated before transmission

Changes department references throughout from "Department of Community Health" to "Department of Health and Human Services" to reflect current state agency structure

Tied to HB 5349 or Senate Bill No. S00334'25 - this bill does not take effect unless one of those companion bills is also enacted into law
