Changes medical exemption certification from "physician" to "health care provider" and requires use of a standardized medical contraindication form created by the department

Requires the department to create both a medical contraindication form and a nonmedical immunization waiver form with specific required fields including vaccine identification, reasons for exemption, and acknowledgment of disease risks

Bill only takes effect if companion legislation (Senate Bill No. ____ or House Bill No. 5348) is also enacted into law
