Annual reports must include overall immunization levels, percentages of children with medical exemptions (section 9215(1)) and non-medical exemptions (section 9215(2))

Requires identification of schools where total immunization exemption rates exceed 5%, along with action plans to reduce exemption rates below that threshold

Children who have not completed required immunizations or filed exemptions must be excluded from school attendance after February 1 (for September enrollees) or within 4 months of enrollment (for mid-year enrollees)
