Adds menopause and perimenopause medications to the existing list of exempt drug categories that cannot require prior authorization, alongside drugs for cancer, HIV/AIDS, epilepsy, mental health conditions, and organ replacement therapy

Applies coverage requirements to both fee-for-service Medicaid and contracts with health maintenance organizations and managed care organizations
