Allows prisoners to designate authorized recipients who can receive their selected medical information upon request, giving incarcerated individuals control over what health information is shared and with whom.

Mandates the department to immediately notify a prisoner's designated emergency contact when the prisoner experiences a critical medical emergency requiring hospitalization.

Permits approved visitors (immediate family members and clergy) to visit hospitalized prisoners during critical medical emergencies, unless the department determines a safety concern exists.

Requires the department to develop the necessary forms within 90 days and create an informational brochure within 60 days of the act's effective date, with the brochure made available on the department's website and at annual health examinations.
