Requires manufacturers to notify the Michigan Department of Environment, Great Lakes, and Energy before selling products containing intentionally added PFAS in categories including apparel, carpets, cleaning products, cookware, dental floss, children's products, menstruation products, and upholstered furniture, effective January 1, 2028.

Mandates products containing intentionally added PFAS carry a label stating "Made with PFAS chemicals" that must be visible before sale and durable for the product's useful life, effective January 1, 2029.

Manufacturer notifications must include chemical abstract service registry numbers, molecular formulas, amount of PFAS by percent weight, and manufacturer contact information for each product category.

Exempts products governed by federal law, used products, medical devices, products manufactured before the act's effective date, and businesses with fewer than 10 employees.

Establishes escalating civil fines for violations: $1,000 per day in the first year, $5,000 per day in the second year, and $10,000 per day (adjusted for inflation) thereafter, enforceable by county prosecutors or the attorney general.
