• Establishes an Early Childhood Community Partnerships grant program to support children's healthy development and school readiness by coordinating access to early care and education services, health services, and family support services from before birth through kindergarten.

• Grant recipients must develop collaborative partnerships among school-based programs, kindergarten teachers, Head Start, child care providers, health care providers, and other community organizations to assess local needs and implement improvement plans.

• Grants are awarded for two-year periods with two types available: planning grants for forming partnerships and implementation grants for direct services, with a 15 percent increase in year two for grantees meeting outcome targets.

• Eligible applicants include school districts, consortiums of school districts, tribal schools, counties, nonprofit organizations, and community-based organizations, with only one grant awarded per geographic area.

• Appropriates unspecified funds from the general fund to the commissioner of education for fiscal year 2012, effective July 1, 2011, with program evaluation required by January 15, 2012.
