Establishes an alcohol use health and safety impact fund in the state treasury to receive revenue from an alcohol use health and safety impact fee.

Requires state commissioners and Minnesota State Colleges and Universities chancellor to certify alcohol use attributable costs by April 30 each year for health care, public health, treatment, and public safety programs.

Imposes an alcohol use health and safety impact fee on distilled spirits at $5.11 per gallon and $1.35 per liter, on wine at $1.55 per gallon and $0.41 per liter, and on fermented malt beverages at $19.84 per 31-gallon barrel, effective July 1, 2010.

Directs the commissioner of management and budget to transfer funds from the impact fund to the general fund to offset certified alcohol-related expenditures, with any remaining balance transferred to the health care access fund.

Requires the commissioner of revenue to certify estimated state payments to local governments for alcohol use attributable costs by April 30 each year, including expenses for court services, law enforcement, corrections, emergency services, and victim assistance.
