Prohibits application of coal tar products on asphalt, concrete, or other pavement surfaces, except as provided in exemptions.

Requires sellers of coal tar products formulated for pavement application to obtain a signed form from purchasers containing the purchaser's name, address, telephone number, purchase date, quantity, a statement that the product will not be used within the state, and an affirmation that the information is correct.

Requires sellers to retain completed purchase forms for not less than three years and allows the Pollution Control Agency commissioner to inspect or copy the forms upon request.

Establishes exemptions when the commissioner determines that a person is researching coal tar effects on the environment, developing alternative technology requiring coal tar products, or no suitable alternative is available for the intended use.

Imposes a civil penalty of up to $1,000 for violations and repeals Minnesota Statutes 2009 Supplement, section 116.201.
