Creates a new medical assistance category for adults without children aged 21-64 who are not pregnant, effective April 1, 2010.

Sets income eligibility for adults without children at 75 percent of federal poverty guidelines, effective January 1, 2011.

Establishes a coordinated care health home program for individuals with chronic conditions, requiring the state plan amendment to be submitted to federal Centers for Medicare and Medicaid Services by January 1, 2011.

Health home services include comprehensive care management, care coordination, transitional care, patient and family support, community referrals, and health information technology coordination.

Appropriates unspecified amounts from the general fund and health care access fund to the commissioner of human services for implementation, effective April 1, 2010.
