Increases the surcharge on health maintenance organizations (HMOs) and community integrated service networks from six-tenths of one percent to 2.5 percent of total premium revenues, effective June 1, 2010.

Imposes a new 2.5 percent surcharge on county-based purchasing plans authorized under section 256B.692, effective June 1, 2010.

Defines "total premium revenue" to include prepaid premiums, Medicare wrap-around premiums, Medicare revenue from service arrangements, and medical assistance revenue, with specific exclusions for Federal Employees Health Benefit Program premiums and federally prohibited Medicare taxes.

Increases payment rates to managed care and county-based purchasing plans under the medical assistance program and MinnesotaCare for families with children by 2.5 percent for services rendered on or after August 1, 2010.

Clarifies surcharge liability in cases of mergers, consolidations, acquisitions, conversions, property transfers, and when entities cease operations.
