Authorizes the commissioner to use volume purchasing through competitive bidding for medical assistance program items including eyeglasses, oxygen, hearing aids, durable medical equipment, nonemergency transportation, drugs, and medical supplies.

Establishes service thresholds and authorization requirements for physical therapy (80 units of CPT codes, 20 modality sessions, 3 evaluations) and occupational therapy (120 units of CPT codes, 2 evaluations) before commissioner authorization is needed for additional services.

Sets reimbursement rates for physical therapy assistants at 100% of physical therapist rates when working under on-premises supervision, and 65% when working under off-premises supervision.

Creates new subdivision for chiropractic services limited to one annual evaluation and 12 visits per year unless prior authorization is obtained for greater services.

Establishes home care therapy requirements including provision in recipient's residence after outpatient therapy access determination, physician prescription, appropriate therapist assessment, and delivery by Medicare-certified home health agencies.
