Establishes HIV Care Coordination Grants program under Minnesota Statutes, section 256B.0757 for providers including nonprofit organizations and public health agencies serving medical assistance recipients with HIV or at risk of contracting HIV.

Grant funds support needs assessments, early intervention services, financial and medical assistance, counseling, home care services, advocacy, and case management services.

Requires reimbursement rate increases and monthly per-patient care coordination fees for providers meeting quality standards including written patient communication protocols, electronic charting, evidence-based HIV treatment guidelines, and systematic tracking of patient outcomes.

Effective dates for reimbursement increases and care coordination fees set for no sooner than July 1, 2011, with commissioner implementation subject to federal approval.

Commissioner authorized to apply for federal grants and demonstrations under the Patient Protection and Affordable Care Act to fund care coordination projects and increase federal match for care coordination fees.
