Creates an alcohol use health and safety impact fund in the state treasury to receive revenue from a new alcohol impact fee.

Imposes an impact fee on all alcoholic beverages: $5.11 per gallon ($1.35 per liter) on distilled spirits; $1.55 per gallon ($.41 per liter) on wine and cider; and $19.84 per 31-gallon barrel on fermented malt beverages, effective July 1, 2010.

Requires state commissioners to certify annual alcohol-attributable costs by April 30 each year for health care, public health, treatment, public safety, and education programs, with local governments certifying costs for court services, law enforcement, corrections, ambulance, hospital, and victim assistance.

Directs the commissioner of management and budget to first transfer fund revenues to the general fund to offset certified state expenditures, then transfer any remaining balance to the health care access fund.
